The first type is the “Affidavit to Replace a Check Not Cashed/Lost or Destroyed by Payee” and the second type is the “Affidavit to Replace a Check Cashed/Lost or Destroyed by Payee”. The first type is used when the check has not been cashed by the payee and the second type is used when the check has been cashed by the payee, but the payee is unable to provide proof.
