The Illinois Petition for Appointment of Guardian for Disabled Person is a legal document used to request the appointment of a guardian for a disabled person who is unable to take care of their own personal and/or financial needs. This document is also used to request the appointment of a guardian for a minor who is unable to care for their own needs. There are two types of Illinois Petition for Appointment of Guardian for Disabled Person: 1) Petition for Appointment of Guardian of the Person; and 2) Petition for Appointment of Guardian of the Estate. The Petition for Appointment of Guardian of the Person requests the appointment of a guardian for the disabled person, who will be responsible for their day-to-day care, such as medical decisions, housing, and basic needs. The Petition for Appointment of Guardian of the Estate requests the appointment of a guardian to manage the disabled person’s financial and legal affairs, such as investments, taxes, and other legal matters.
