Indiana Notice to Court-Appointed Counsel of Public Disclosure of Attorney Fee Information is a form that must be filled out by court-appointed counsel in order to disclose to the public information about attorney fees. This form is provided by the Indiana State Bar Association and must be completed and filed with the court. There are two types of Indiana Notice to Court-Appointed Counsel of Public Disclosure of Attorney Fee Information: 1) Non-Final Disclosure and 2) Final Disclosure. The Non-Final Disclosure form is used when the court-appointed counsel is still in the process of negotiating a fee. This form must be submitted to the court prior to entering into a contract with the client. The Final Disclosure form is used when the court-appointed counsel has already entered into a contract with the client and the fee has been set. This form must be submitted to the court after the contract is finalized. Both forms require the attorney to include detailed information about the attorney's services, the fees charged, and any other pertinent information.
