The form must be completed by the court and must include information such as the name of the defendant, the name of the attorney, and the amount of the attorney’s fees. The form must be signed by both the court and the attorney in order to be valid. Once the form is completed and signed, the court will issue an order authorizing payment of the attorney’s fees, which will be paid out of the court’s funds.
