In addition to the affidavit, the parent or guardian of the minor must provide a certified copy of the minor's birth certificate and the minor's social security card. In addition, the Adult Name Change Affidavit must include the individual's date of birth, address, and driver's license number. The Minor Name Change Affidavit must include the parent or guardian's name, address, and driver's license number.
