Indiana Corporate Ownership Statement-Adv is a form that must be completed and filed with the Indiana Secretary of State when a corporation is formed in the state. It is used to provide basic information about the corporation, its owners, and the registered agent who will accept service of process for the corporation. The form is also used to confirm that the corporation is organized according to Indiana law and is in compliance with all applicable laws and regulations. There are three types of Indiana Corporate Ownership Statement-Adv forms: Domestication, Amendment, and Dissolution. The Domestication form is used when an existing corporation in another state wishes to form a new corporation in Indiana. The Amendment form is used when changes are made to the existing corporation’s information, such as the name, address, or ownership structure. The Dissolution form is used when the corporation is dissolved.
