The Indiana Prisoner Complaint Form (Judicial Misconduct Or Disability) is a form used to report any misconduct or disability of a judge or court personnel in the state of Indiana. This form is used to alert the Indiana Judicial Qualifications Commission (JC) when an individual believes a judge has acted inappropriately or is unable to perform their duties. There are two types of Indiana Prisoner Complaint Form (Judicial Misconduct Or Disability): one for judicial misconduct and one for disability. The judicial misconduct form should be used to report any behavior or action of a judge that is in violation of the Indiana Code of Judicial Conduct, which sets standards for judges’ professional and ethical conduct. This form should also be used to report any conflict of interest or violation of the law. The disability form should be used to report any disability of a judge, such as physical or mental impairment, that prevents them from carrying out their duties and responsibilities. Both forms require detailed information regarding the complaint, such as the judge’s name, description of the alleged misconduct or disability, and any other relevant information. Upon receiving the complaint, the JC will investigate the matter and decide whether disciplinary action should be taken.
