The Kentucky Application for Hardship Driver's License (Restitution Owed -Theft-Related Offense) is a form used by an individual to apply for a restricted driver's license in Kentucky due to a financial hardship caused by a restitution owed for a theft-related offense. This form is used by individuals who have had their license suspended or revoked due to a restitution debt for a theft-related offense, and are seeking a restricted license in order to be able to drive to work or school and meet other basic needs. There are two types of Kentucky Application for Hardship Driver's License (Restitution Owed –Theft-Related Offense): 1. The Hardship Driver's License Application — Restitution Owed for Theft-Related Offense (Form TC 94-122). This form must be completed and submitted to the Kentucky Department of Transportation in order to apply for a restricted driver's license due to a restitution debt for a theft-related offense. 2. The Hardship Driver's License Verification of Restitution Owed for Theft-Related Offense (Form TC 94-123). This form must be completed and submitted to the Kentucky Department of Transportation in order to verify that the applicant has met the requirements for a restricted driver's license due to a restitution debt for a theft-related offense.
