The Kentucky Application for Appointment of Fiduciary for Disabled Persons is a form used by the state of Kentucky to appoint a fiduciary to take over the care of an adult with a disability. This form is used to provide legal authority to an appointed fiduciary to manage the personal and financial affairs of the disabled person, as well as to provide protection from exploitation and abuse. There are four different types of Kentucky Application for Appointment of Fiduciary for Disabled Persons: the Application for Appointment of a Fiduciary to Make Health Care Decisions, the Application for Appointment of a Fiduciary to Manage Personal Affairs, the Application for Appointment of a Fiduciary to Manage Financial Affairs, and the Application for Appointment of a Fiduciary to Manage Both Financial and Personal Affairs. Each application has its own application form and must be completed in order for the fiduciary to be appointed.
