The Kentucky Financial Statement, Affidavit of Indigence, Request for Counsel and Order (DNA-TPR Cases) is a set of documents used in the state of Kentucky for DNA-TPR (Termination of Parental Rights) cases. The Kentucky Financial Statement is used to determine whether a parent is able to pay the costs associated with the DNA test, and if not, the parent can file an Affidavit of Indigence to waive those fees. The Request for Counsel and Order is then issued to the court to provide the parent with an attorney to represent them in the proceedings. There are three different types of Kentucky Financial Statement: 1) Standard Financial Statement; 2) Modified Financial Statement; and 3) Automated Financial Statement. The Standard Financial Statement is used when the parent has sufficient income to cover the costs associated with the DNA testing. The Modified Financial Statement is used when the parent has limited income and cannot afford the associated costs. The Automated Financial Statement is used when the parent has no income or very limited income.
