The Kentucky Notice and Affidavit of Foreign Judgment Registration is a form required by the state of Kentucky in order to register a foreign judgment for collection. The notice and affidavit must be filed with the clerk of the court in the county in which the debtor lives or owns property. The form includes a section for the creditor to provide information such as the name and address of the debtor, the name and address of the court where the judgment was obtained, the date of the judgment, the amount of the judgment, and any interest and costs due. The form also requires an affidavit from the creditor attesting to the accuracy of the information provided. There are two types of Kentucky Notice and Affidavit of Foreign Judgment Registration: Original Notice and Renewal Notice. The Original Notice is used to register the foreign judgment for collection in the state. The Renewal Notice is used to renew the judgment for an additional five years.
