The Kentucky Notice of Appeal and Statement of Election is a document used to appeal a decision made by the Kentucky Administrative Office of the Courts. It must be filed by the appellant within 30 days of the date of the decision in order to begin the process of appealing the decision to a higher court. The Notice of Appeal must include the name of the court from which the appeal is being taken, the identity of the appellant and respondent, the date of the decision being appealed, and a concise statement of the facts supporting the appeal. The Statement of Election is a separate document that must be filed with the Notice of Appeal. This document is used to indicate which appellate court the appellant wishes to have heard their appeal. There are three types of Kentucky Notice of Appeal and Statement of Election: a Notice of Appeal to the Kentucky Supreme Court, a Notice of Appeal to the Kentucky Court of Appeals, and a Notice of Appeal to the Kentucky Circuit Court.
