The Kentucky Statement of Military Service is a document that certifies an individual's honorable discharge or release from active duty in the United States Armed Forces. It is issued by the Kentucky Department of Veterans Affairs (DVA) and is required for any benefits related to military service in the Commonwealth of Kentucky. The Kentucky Statement of Military Service is available in three forms: DD Form 214, GB Form 22, and the Kentucky Certificate of Honorable Discharge. The DD Form 214 is the official record of military service and is issued upon separation or retirement from active duty. The GB Form 22 is issued to members of the National Guard or Reserve components upon discharge or release from active duty. Lastly, the Kentucky Certificate of Honorable Discharge is issued to veterans who served in the Kentucky National Guard and who were honorably discharged or released from active duty. All documents must be signed and notarized to be considered valid.
