The Kentucky Bankruptcy Petition Preparer's Notice, Declaration and Signature is a set of documents that must be completed by a bankruptcy petition preparer in order to be authorized to assist with bankruptcy filings in the state of Kentucky. This document is required in order to protect the person filing for bankruptcy and to ensure the accuracy of the information in the filing. It includes the Petition Preparer's Notice, which informs the debtor of the services provided and any fees associated; the Petition Preparer's Declaration, which states that the petitioner has been given the necessary information and that all the information that has been provided is true and correct; and the Petition Preparer's Signature, which confirms the petitioner's agreement to the terms and conditions of the notice. There are two types of Kentucky Bankruptcy Petition Preparer's Notice, Declaration and Signature: the Basic and the Advanced. The Basic Notice, Declaration and Signature is used for basic bankruptcy filing services, while the Advanced Notice, Declaration and Signature is needed for more complex services, such as chapter 13 or chapter 11 bankruptcy.
