The Kentucky Request for Excuse is a form issued by the Kentucky Board of Education allowing students to request an excused absence from school. This form must be completed by the student's parent or guardian and submitted to the school principal or the school district's attendance officer. The Request for Excuse must include the student's name, the date of the absence, the reason for the absence, and the parent's signature. It also requires the school to provide documentation from a doctor or other medical personnel if the student is absent due to illness. There are two types of Kentucky Request for Excuse forms: the Short-Term Request for Excuse and the Long-Term Request for Excuse. The Short-Term Request for Excuse is used for absences of up to three consecutive days. The Long-Term Request for Excuse is used for absences of more than three consecutive days. Both forms must be approved by the school principal or the school district's attendance officer before the absence can be excused.
