There are two types of Kentucky Advice of Penalties-Acknowledgment forms: one for individuals and one for businesses. The form includes information about the tax period, the taxpayer's name, address, and Social Security Number, the type of taxes owed, the amount due, the applicable penalties, and the potential interest.
