The Kentucky Non-Wage Garnishment Form 2014 is a document used by creditors in Kentucky to collect a debt from a debtor. This form is used to garnish a debtor’s non-wage assets, such as a bank account, stock, or investment account. It must be completed in full, including the debtor’s information, the creditor’s information, the amount to be collected, and the type of asset or property to be garnished. There are three types of Kentucky Non-Wage Garnishment Forms 2014: the Bank Levy Form, the Stock/Investment Account Levy Form, and the Other Asset Levy Form. All three forms are available for download on the Kentucky Department of Revenue website.
