The Kentucky Request for Compact Disc (CD) of Audio Recording of Court Proceeding is a document used to obtain a CD containing audio recordings of court proceedings from courts in the state of Kentucky. The request must be made in writing and include the name of the court, the case number, the name of the judge, the date of the hearing, and the name of the party requesting the audio recording. The request must also include the name and address of the party requesting the audio recording, as well as the title of the recording and the type of audio format desired.
