Kentucky Average Weekly Wage Certification — Post Injury is a form used to certify the average weekly wage of an injured worker who is seeking benefits from the Kentucky Department of Workers’ Claims. The form is used to calculate the payment of benefits and must be completed by the employer of the injured worker. The form includes the name of the injured worker, the date of injury, the employer's contact information, the total wages earned in the 52-week period prior to the date of injury, the total weeks worked during the 52-week period, the number of hours worked per week, and the total gross wages. There are two types of Kentucky Average Weekly Wage Certification — Post Injury forms: one for employees who are paid hourly and one for employees who are paid a salary. Both forms must be completed in order for the injured worker to receive the correct amount of benefits.
