This is an example of a father consenting to the adoption of his child by the husband of the child's mother, and to the change of the child's last name to that of the stepparent. The biological father also waives his rights to be served with a copy of the petition. An act of consent to adoption is a sworn statement of consent signed by the person, such as a parent or guardian, who is relinquishing rights with regard to a minor, for the purposes of adoption by a named person(s), in this case by a stepparent. The consent must be signed and acknowledged before a notary. In Louisiana, both biological parents of the child must consent to the adoption. See La. C.H.C. Article 1193.
