A consent or permission to participate in program and data collection concerning a universal, social, and behavioral screening to all students in an effort to assess any impact the COVID-19 pandemic may have on student well-being and if necessary provide social, emotional, and/or behavioral services. Further consent will be required to receive any of the social, emotional, and/or behavioral services. The consent provides assurance of confidentiality pursuant to the Family Educational Rights and Privacy Act (FERPA) and requires your consent pursuant to the Protection of Pupil Rights Amendment (PPRA) in order to participate in the screening process.
