It also contains information about the court, the type of action being brought, the cause of action, and any special damages sought. There are three types of Louisiana Civil Cover Sheet: the Joint Petition for Divorce, the Separate Maintenance Cover Sheet, and the General Civil Cover Sheet. The Joint Petition for Divorce must be completed when the parties are seeking a divorce. The Separate Maintenance Cover Sheet is used when the parties are seeking a legal separation. The General Civil Cover Sheet is used when the parties are not seeking a divorce or legal separation, but instead are seeking an action such as contract enforcement or wrongful death.
