The Louisiana Request to File Notice of Termination and/or Acceptance of Work is a form used by contractors and subcontractors to file a notification with the state of Louisiana informing them of a job completion or termination. This document is used to terminate or accept a contract between a contractor and a subcontractor. The form must be signed by both parties, and must include the legal name and address of each party, the date of the notice, and a detailed description of the work that was completed or terminated. There are two types of Louisiana Request to File Notice of Termination and/or Acceptance of Work: Notice of Termination and Notice of Acceptance. The Notice of Termination is used when a contractor is terminating a contract with a subcontractor, while the Notice of Acceptance is used when a contractor is accepting the completion of a contract with a subcontractor. Both documents must be filed with the state of Louisiana within 30 days of the completion or termination of the contract.
