Louisiana Confidential Address Form is a document used to protect the identity of individuals who are victims of domestic violence, sexual assault, human trafficking, stalking, or other situations in which their safety may be at risk. It replaces a person's physical address with a confidential address, usually a post office box, that can be used to receive mail and other correspondence. There are two types of Louisiana Confidential Address forms: the Confidential Address for Victims of Domestic Violence Form and the Confidential Address for Victims of Human Trafficking Form. Both forms are available online and are issued by the Louisiana Attorney General's Office. The Confidential Address for Victims of Domestic Violence Form is used to protect victims of domestic violence from being identified by their abusers. The Confidential Address for Victims of Human Trafficking Form is used to protect victims of human trafficking and other vulnerable individuals from being identified by their traffickers. Both forms provide a confidential address for the person to use in all of their legal and financial dealings, including for receiving mail and other correspondence.
