Louisiana Marriage Officiant Registration Affidavit is a form that must be completed and filed with the Louisiana Vital Records Registry in order for an individual to become a legally registered officiant of marriages in Louisiana. The form is used to verify the individual's identity, as well as any previous marriages officiated, and the individual's qualifications to be an officiant. There are two types of Louisiana Marriage Officiant Registration Affidavit: the Louisiana Marriage Officiant Registration Affidavit (MORE) and the Louisiana Marriage Officiant Registration Affidavit (Morals). The MORE form is used for individuals who have been ordained or licensed by a recognized religious or spiritual organization, while the Morals form is used for non-ordained individuals who have been appointed by a court of law. Both forms require the individual to provide information such as their name, address, phone number, and social security number. The forms also require the individual to provide a letter from the religious organization that ordained or licensed them, or a court order appointing them to officiate marriages. The forms must then be signed by the applicant, notarized, and submitted to the Louisiana Vital Records Registry.
