Massachusetts Health Care Proxy is a document that allows individuals to appoint a person of their choice, called a Health Care Agent, to make health care decisions on their behalf should they become incapacitated and unable to make decisions for themselves. This document is intended to help individuals maintain control of their health care decisions even if they become unable to make decisions for themselves. There are two types of Massachusetts Health Care Proxy: Durable Power of Attorney for Health Care and Declaration to Physicians. Durable Power of Attorney for Health Care is a legal document that gives an individual's health care agent the authority to make decisions about their medical care and treatment. Declaration to Physicians is a document that allows individuals to provide specific instructions about their medical care and treatment, and to designate a health care agent to make decisions on their behalf if they become unable to make decisions for themselves.
