The MFR is completed by a qualified professional who is responsible for requesting and authorizing interpreter services. The MFR includes basic information such as the individual's name and contact information, the language and dialect that needs to be interpreted, the type of interpreting service needed, and the date and location of the interpretation. There are two types of Mfrs: a one-time request and a long-term request. The one-time request is for a single interpretation event, while the long-term request is for multiple interpretation events over a period of time.
