The Massachusetts Military Affidavit for E-filing is a document used by service members in the Massachusetts state to provide evidence of their military service. It is used for various legal proceedings, such as requesting exemptions from jury duty, court proceedings, or other legal matters. The affidavit must be signed by the service member and/or a commanding officer, and must include the service member’s name, rank, service number, and date of service. There are two types of Massachusetts Military Affidavit for E-filing: the Limited Power of Attorney and the Full Power of Attorney. The Limited Power of Attorney is used when the service member is not able to appear in court in person, but wants to provide evidence of their service. The Full Power of Attorney is used when the service member is able to appear in court in person but requires legal representation.
