The Massachusetts Civil Cover Sheet is a form used to provide the court with information about a civil case. This document is required to initiate any civil case in the state of Massachusetts. The cover sheet must be filed with the court when filing a complaint, answer, or any other civil pleading. There are two types of Massachusetts Civil Cover Sheet: 1) Standard Civil Cover Sheet and 2) Complex Civil Cover Sheet. The Standard Civil Cover Sheet is used to initiate a civil action in the District Court, Superior Court, Housing Court, Probate and Family Court, and Land Court. The Complex Civil Cover Sheet is used to initiate a civil action in the Superior Court, Land Court, and Supreme Judicial Court. The Massachusetts Civil Cover Sheet contains information such as: the name and address of the plaintiff and defendant, the types of relief sought, any discovery disputes, any pending or related cases, and the estimated value of the case. The information provided on the form helps the court to determine what type of action should be taken.
