The Massachusetts Clinical Team Report (MTR) is a comprehensive report that provides information about a patient’s physical, behavioral, and psychological health. The Comprehensive MTR is a more comprehensive version which includes detailed information about a patient’s physical, behavioral, and psychological health. The Standard MTR is a shorter report that only includes essential information about the patient’s medical history, physical examination, laboratory results, and medications.
