The Massachusetts OR Provider Quarterly Report for Closed Cases — RTW is a report used by Vocational Rehabilitation (VR) providers to document the successful return to work (RTW) of an individual with a disability who has received services from the provider. The report contains information about the services received, outcomes achieved, and RTW status for cases closed during the previous quarter. There are two versions of the report, one for employers and one for individuals. The employer version includes data on the services and outcomes for the closed cases, the RTW status of the individual, and the total cost of services. The individual version includes data on the services and outcomes for the closed cases, the RTW status of the individual, and any other supports or services used to achieve the successful RTW. Both versions of the report are used to inform VR providers about the effectiveness of their services and the success of their RTW efforts.
