The Maryland Americans With Disabilities Act (ADA) Grievance Form (Bilingual — Russian Format) is a document used to file a complaint or grievance about an alleged violation of the ADA. It is available in both English and Russian, as the ADA protects the rights of people with disabilities in the United States regardless of race, color, national origin, sex, age, or disability. The form can be used to document any ADA-related complaints or grievances about discrimination against people with disabilities in employment, public accommodations, and other areas of life. There are two types of Maryland ADA Grievance Forms (Bilingual — Russian Format): the Private Employer Complaint Form and the Public Accommodations Complaint Form. The Private Employer Complaint Form is used for filing complaints about alleged ADA violations in the workplace, and the Public Accommodations Complaint Form is used for filing complaints about alleged ADA violations in areas such as transportation, hotels, restaurants, and public venues. Both forms are available online for free download.
