Maryland Request for Accommodation by Persons with Disabilities (Bilingual — Russian Format) is a form issued by the Maryland Department of Disabilities (MOOD) to allow individuals with disabilities to request accommodations in order to access services and programs provided by the MOOD. This form can be completed in either English or Russian and is available in both digital and print formats. There are two types of Maryland Request for Accommodation by Persons with Disabilities (Bilingual — Russian Format): (1) Request for Accommodation Form and (2) Request for Reasonable Accommodation Form. The Request for Accommodation Form is used to request modifications to MOOD services and/or programs, such as changes to existing physical or communication barriers that impede access. The Request for Reasonable Accommodation Form is used to request changes to existing policies or procedures that impede access, such as requesting an interpreter, special seating arrangements, or other reasonable accommodation. Both forms must be completed and submitted to the MOOD for review. Upon review, the MOOD will determine if the request can be accommodated and if so, will provide an approved accommodation.
