It contains information such as the project name, the contractor’s name and address, the total amount of open costs, the date of the request, and the contractor’s signature. The form must be completed and signed by the contractor in order to be considered for a waiver. There are two types of Maryland Request for Final Waiver of Open Costs (Bilingual — Chinese Format): The Application for Final Waiver of Open Costs and the Request for Final Waiver of Open Costs. The Application form is used when a contractor is requesting a final waiver for a project and the Request form is used when a contractor is requesting a final waiver for a portion of a project.
