The Maryland Statement of Social Security Number is also used as a valid form of identification for certain government-related transactions, such as accessing public benefits and services, opening bank accounts, and obtaining a driver's license. The Maryland Statement of Social Security Number is available in two forms: the Standard Form and the Reduced Form. The Standard Form includes the individual's name, Social Security Number, date of birth, address, and signature. The Reduced Form includes only the individual's name and Social Security Number. Both forms are issued on a card-like document that is valid for three years from the date of issuance.
