It also requires the prospective guardian to provide a list of references, including two non-related individuals who can attest to the prospective guardian's character and ability to fulfill the duties of a guardian. There are two types of Maryland Prospective Guardian Information Sheet: the Adult Guardian Information Sheet and the Minor Guardian Information Sheet. The Adult Guardian Information Sheet is used for an adult who is requesting to be appointed as guardian of the minor, whereas the Minor Guardian Information Sheet is used for a minor who is requesting to be appointed guardian of himself/herself or another minor.
