The Maryland Statement of Wage Information (also known as the SOW) is a form issued by the Maryland Department of Labor, Licensing and Regulation (DLR) to document wages earned for a specific period of time. It includes information on an employee's wages, hours worked, deductions, and taxes. The SOW is used by employers to determine the amount of taxes to be withheld from an employee's paycheck. There are two types of Maryland Statement of Wage Information: the Standard SOW and the Request for Additional SOW. The Standard SOW includes information on the wages earned during a specific payroll period, the hours worked during that period, the deductions taken from the employee's wages, and the taxes withheld. The Request for Additional SOW is used when an employee has multiple jobs or when the employer needs to make corrections to the original SOW. The Request for Additional SOW includes the same information as the Standard SOW, but it also includes additional information such as the employee's Social Security number, address, and date of hire.
