The Maryland Workers Compensation Employers Questionnaire (Uninsured Employer) is a questionnaire for employers who are not covered by a Maryland Workers Compensation plan. This questionnaire is designed to determine if an employer is required to register for and pay into a Maryland Workers Compensation plan. There are two versions of the Maryland Workers Compensation Employers Questionnaire (Uninsured Employer): one for employers with 1-4 employees and one for employers with 5 or more employees. The questionnaire includes questions about the employer's business name and address, the type of business, the number of employees, the type of work performed, and the amount of payroll. The questionnaire also asks for information about the employer's Workers Compensation insurance coverage, including the name of the insurer, the policy number, and the date of the policy. The questionnaire is used to help the Maryland Workers Compensation Commission determine if an employer is required to register for and pay into a Maryland Workers Compensation plan. If an employer is found to be required to register, they will receive a notice from the Commission informing them of their obligation.
