The Maryland Department of Health and Mental Hygiene Certificate of Adoption Form is a document issued by the Maryland Department of Health and Mental Hygiene (DHH) that provides evidence of a legal adoption in the state of Maryland. It is used to change the legal name of the adopted child and to obtain a new birth certificate. There are three types of Maryland Department of Health and Mental Hygiene Certificate of Adoption Forms: Certificate of Adoption by a Court Order, Certificate of Adoption with No Court Order, and Certificate of Adoption by a Private Agency. The Certificate of Adoption by a Court Order is used when the court has issued an adoption order. The Certificate of Adoption with No Court Order is used when the final adoption order has not yet been issued. The Certificate of Adoption by a Private Agency is used when a private agency has finalized the adoption. All forms must be completed in full and signed by the adopting parent(s).
