It allows parents to complete the application online and submit it electronically. 2. The Paper Application — This is the paper version of the online application. It is available at your local Friend of the Court office or online. 3. The Referral Form — This is used to refer another parent or guardian to the Friend of the Court office for child support services. It must be completed and signed by both parties and can be submitted at your local Friend of the Court office or online.
