Michigan Release of Information Authorization Adult Adopted is a document that is required in order for an adopted adult to access their adoption records. This document is used to authorize the release of information from the adoption agency or court to the adopted adult. Depending on the circumstances, there are two types of Michigan Release of Information Authorization Adult Adopted forms: the original birth certificate form and the amended birth certificate form. The original birth certificate form is used when a person was born in Michigan and adopted in Michigan, and the amended birth certificate form is used when a person was born outside of Michigan and adopted in Michigan. Both forms require the signature of the adopted adult and the signature of a witness. The Michigan Release of Information Authorization Adult Adopted also requires the Social Security Number of the adult adopted. Once the form is completed and signed, it must be submitted to the court or adoption agency in order to access the adoption records.
