Michigan Application for Reimbursement (MAR) is a state-specific form used by Michigan residents to apply for reimbursement of certain medical expenses. It is issued by the Michigan Department of Health and Human Services (MD HHS). There are two types of MAR forms: Standard MAR and Special MAR. The Standard MAR is used for reimbursement of medical expenses incurred for services provided in-state, including hospital stays, doctor visits, laboratory tests, prescription drugs, and durable medical equipment. It is also used for reimbursement of private health insurance premiums. The Special MAR is used for reimbursement of medical expenses incurred out-of-state, such as out-of-state hospital stays, doctor visits, laboratory tests, prescription drugs, and durable medical equipment. It is also used for reimbursement of out-of-state private health insurance premiums. In order to be eligible for reimbursement, the medical expenses must be deemed medically necessary by an approved healthcare provider. The MAR form must be completed and submitted to the MD HHS for review. Once approved, the applicant will be reimbursed for eligible expenses.
