The Michigan Support Complaint Form is a document used by individuals who wish to file a complaint against the Michigan Department of Health and Human Services for failing to provide necessary support. This form is used to file complaints concerning the following topics: failure to provide timely payments; failure to provide correct payments; failure to provide accurate information; improper or inadequate service; and other issues. The form is divided into two parts: Part A is for general information and Part B is for providing details regarding the complaint. There are three types of Michigan Support Complaint Form: Part A, Part B, and Part C. Part A is used to provide contact information and to describe the issue in general. Part B is used to provide specific details concerning the complaint. Part C is used to provide additional information that may be relevant.
