The Michigan Probate Court Request for Accommodations Form is a legal document used to request accommodations from the Michigan Probate Court. This form is used by individuals who are unable to attend or participate in a court proceeding due to a disability. The form is used to request reasonable accommodations such as an interpreter, a wheelchair accessible entrance, a Braille copy of the court documents, or other assistance in order to enable the individual to attend and/or participate in the court proceedings. There are two types of Michigan Probate Court Request for Accommodations forms: the General Request for Accommodations Form and the Request for Mental Health Related Accommodations Form. Both forms require the name, address, phone number, and email address of the individual requesting the accommodation. The General Request for Accommodations Form also requires a description of the requested accommodation and a brief description of the individual’s disability. The Request for Mental Health Related Accommodations Form requires the name of the individual’s mental health provider, a description of the requested accommodation, and a brief description of the individual’s mental health condition. Both forms must be signed by the individual requesting the accommodation and submitted to the Michigan Probate Court.
