The Michigan Request to be Excused from Early Intervention Conference (EIC) is a form that parents, guardians, or other legally responsible persons can submit to the local Early On Michigan program in order to request an excuse from attending an EIC. The form is broken down into two types: a Request for an Excuse from Core EIC and a Request for an Excuse from Additional EIC. The Request for an Excuse from Core EIC is used when the parent/guardian is unable to attend the initial EIC meeting due to a conflict with their schedule or other extenuating circumstances. This form must be submitted at least five days before the scheduled EIC meeting. The Request for an Excuse from Additional EIC is used when the parent/guardian is unable to attend any additional EIC meetings beyond the initial meeting due to a conflict with their schedule or other extenuating circumstances. This form must be submitted at least five days before the scheduled additional EIC meeting. Both forms require the parent/guardian to provide full contact information, information about the child, the reason for the request, and any additional information that can aid the local Early On Michigan program in making their decision. Once the form is submitted, the local Early On Michigan program will review the request and make a decision regarding the excuse.
