The Michigan Certificate of Stillbirth is a legal document issued by the state of Michigan, in accordance with the Michigan Vital Records Act (Public Act 368 of 1978). It is used to record the stillbirth of a child and serves as a permanent record of the event. This document can be requested by the parent(s) of the stillborn child, or by the attending physician. There are two types of Michigan Certificate of Stillbirth: a Certificate of Stillbirth (Form 2368) and a Certificate of Fetal Death (Form 2368 (A)). The Certificate of Stillbirth is used to register a stillbirth that occurred in the state of Michigan. It contains identifying information about the stillborn child, such as the name, date of birth, gender, and place of birth. It also includes information on the parent(s) of the stillborn child. The Certificate of Fetal Death is used to register a stillbirth that occurred outside the state of Michigan. It contains similar information to the Certificate of Stillbirth, but also includes information about the place of death. The Michigan Certificate of Stillbirth is an important legal document that serves as a permanent record of a stillbirth and provides the necessary information for the proper registration of the stillbirth.
