There are two types of Michigan Insurer's Notice of Issuance of Policy (fill-in form): an Automobile Notice of Issuance of Policy and a Health Notice of Issuance of Policy. The Automobile Notice of Issuance of Policy includes the make and model of the vehicle, the Vehicle Identification Number (VIN) and the type of coverage provided. The Health Notice of Issuance of Policy includes the name of the insured, the type of coverage provided, the deductible and the co-payment amounts. Both forms must be completed and signed by both the insurer and the insured.
