The Mn SCU Transcript Request Form is used to request transcripts from any of the 31 state universities, community and technical colleges that are part of the Mn SCU system. The University of Minnesota Transcript Request Form is used to request transcripts from the University of Minnesota Twin Cities, Duluth, Morris, and Crooks ton campuses. Both forms require the student's personal information, contact information, academic information, and the name and address of the recipient of the transcript.
