The Minnesota Abandoned Name Affidavit is a legal document used to change a person’s name. It is used when the person has had a name that has been abandoned or forgotten for at least five years. The affidavit must be completed and signed by the person requesting the name change and submitted to the Minnesota Department of Health. There are two types of Minnesota Abandoned Name Affidavits: an Adult Affidavit and a Minor Affidavit. The Adult Affidavit requires the signature of the person requesting the name change, two witnesses, and a notary public. The Minor Affidavit requires the signature of the parent or guardian and one witness. Both affidavits must include the current and requested name, the reason for the name change, and the place and date of birth. The affidavit must also be accompanied by a copy of a valid government-issued photo ID and a certified copy of the applicant’s birth certificate. Once the affidavit is submitted, a hearing will be scheduled and a court order may be issued.
