The Minnesota Affidavit/Proof of Transfer of Firearms is a form used to document the transfer of a firearm from one owner to another. This form is used for any transfer of ownership of a firearm that is not done through a licensed firearms' dealer. It is required by Minnesota State Law when a firearm is transferred for any purpose. There are two types of Minnesota Affidavit/Proof of Transfer of Firearms forms: one for long guns (e.g., rifles and shotguns) and one for handguns. Each form requires that the transferor and transferee provide the following information: name, address, date of birth, driver's license number, and signature. The transferor must also provide the make, model, and serial number of the firearm. The form must be signed by both parties and notarized. The form is then filed with the local law enforcement agency, along with a copy of the transferor's driver's license. The form must be submitted within seven days of the transfer.
